Forget-Me-Not-Program for A.L.F.s

The WPH & CC is now offering the Forget-Me-Not-Program in Assisted Living Facilities (ALF). WPH&CC can bring several of our services directly to the client. With aging, pelvic support disorders including bladder control problems and incontinence, prolapse and anal incontinence become more symptomatic requiring more assistance and expense. Incontinence problems become increasingly expensive due to extra hygiene care, higher facility expense levels, and increased costs of health care due to higher rates of infections, skin rashes, bedsores and increased risk of falling. Incontinence is the second most common reason for admission into a Long Term Care facility.

Our program can mean a considerable overall cost savings for our assisted living residents. Our proactive incontinence program is covered 80% by Medicare and 100% with secondary insurance. Physical Therapy visits are 100% covered. Inclusion in the program will never increase their expenses at the facility; our goal is to decrease their expense and improve overall quality of life. If your ALF facility does not offer this program please contact your Executive Director.

ALF residents can request these services for themselves or families can request it for their loved one. During the initial stage clients will be given a questionnaire that will provide us with information on their problems. During the initial visit an exam with simple testing will be done along with biofeedback. The results will be recorded and review by a trained Physician Assistant or Nurse Practitioner. The client will then be instructed in pelvic floor exercises and will have follow-up visits by certified physical therapists that will assist them in their rehab. Weekly Forget-Me-Not-Aerobics classes will be held in most facilities. Follow-up monthly biofeedback visits will measure improvement. If desired results aren't achieved an appropriate referral may be made or further work-up suggested.
